We are currently seeking a dynamic SERVICE COORDINATOR to join our team at our office in Kapolei. The Service Coordinator will be responsible for providing service coordination services for QUEST Integration members with behavioral health special health care needs. Responsibilities include screening, assessing, and managing of members referred for service coordination. The Service Coordinator is responsible for providing outreach, coordination, education and support to HMSA members by performing clinically based activities to coordinate care between the members, providers and community resources. Responsibilities also include the facilitation of a collaborative relationship among members, providers, community agencies, as well as health plan and Beacon staff in order to ensure members receive timely and integrated access to services across the continuum of care, including supporting and managing member’s physical, emotional, social and economic barriers to appropriate medical and behavioral care.

Position Responsibilities:

Screening and tracking of all assigned SC Members Conduct timely face to face health and functional assessments and re-assessment of members in compliance with program standards Develop, document, and implement targeted service coordination service plans based on the results of the assessment for members with behavioral health special health care needs. Maintains on-going contact with the member/guardian Document and resolve issues for HMSA QUEST Integration members and providers on appropriate electronic systems. Provide continuity of care when members are discharged from a hospital and help them with medication adherence and reconciliation. Utilize compiled data received from member encounters to assure the services provided meet the member’s needs.

Position Requirements:

Education:

Staff providing Service Coordination services will have at least one of the following:

Master’s degree or higher in a health related field and licensure as a behavioral health professional (LSW or LCSW)

RN licensure as a professional and 3 years of behavioral health clinical experience.

Licensure:

Relevant Work Experience:

Clinical experience working with children, adolescents and families with emotional and behavioral disturbances. Possess knowledge of developmental psychology and child and adolescent psychosocial interventions.

3 plus years of Case Management experience

Knowledge, Skills & Abilities:

Educated in current principles, procedures and knowledge domains of case management based on nationally recognized standards of case management

Knowledge of HIPAA and other privacy regulations

Knowledge of client's rights, state and federal laws, and regulations, such as those relating to health services, confidentiality, and consent issues for the targeted population.

Ability to prioritize and manage multiple projects and assignments.

Ability to seek help in managing workload, when appropriate.

Ability to work independently, handle multiple assignments and prioritize workload.

Driving/Travel requirements:

Requires safely operating an insured automobile for travel to off-site locations to conduct and accomplish business related activities. Must have current and valid driver’s license, access to an automobile with current registration and no fault insurance.

Able to travel to off-site meetings with members, providers and community agencies. Must be able to transport self and equipment to community sites, member homes, physician offices etc. May require ability to travel to Neighbor Islands and flexibility in being available to work after hours and on weekends when needed.
