Primary Care Physician - Queen's Health Systems (Hawaii)

RESPONSIBILITIES

I. JOB SUMMARY/RESPONSIBILITIES:

Provides the basics of health care, focusing on wellness and prevention.

Focuses on the patient’s overall health. Ensures the patient receives recommended screenings and risk assessments.

Demonstrates a broad clinical knowledge and attention to detail necessary to cover all possible diagnoses in a limited time.

Demonstrates excellent communications skills, listening skills, and the ability to extract the pertinent information from patients who are sick or injured and not at their best.

Demonstrates compassion, patience, and engagement in shared decision-making with patients regarding treatment choices and goals of care.

Makes appropriate referrals to specialists, while continuing to manage the overall care of patients and communicating with specialists as needed.

Demonstrates the ability to transform practice from individual patient management to population health management by embracing team-based management and alternative visits through telehealth, telephone, email, etc.

Assignment to Primary Care Physician Enterprise (PCPE):

Provides care in a team-based model with a focus on preventive medicine, complex care management, transitional care management, chronic disease management, and acute illness visits.

Demonstrates philosophical alignment with patient-centered, team-based care. They should have a commitment to patient-centered approaches to healthcare delivery, along with strong relationship building, communication, and active listening skills.

Actively seeks and appropriately responds to patients’ preferences and values, and works with the entire multidisciplinary care team to support patients in achieving their goals. Understands, values, and optimizes the roles of each member of the health care team, and works collaboratively with team members, patients, and their caregivers to achieve coordinated, high-quality care.

II. TYPICAL PHYSICAL DEMANDS:

Essential: finger dexterity, seeing, hearing, speaking.

Continuous: sitting, static gripping of an object for prolonged periods.

Frequent: walking.

Occasional: standing, stooping/bending, climbing stairs, walking on uneven ground, lifting and carrying weight up to 20 pounds, reaching above, at and below shoulder level, frequent gripping of an object.

Operates computer, calculator, telephone, fax and printer.

III. TYPICAL WORKING CONDITIONS:

Works in an ambulatory setting with a team of medical professionals providing patient care.

Not substantially subjected to adverse environmental conditions.

Exposure to hazardous chemicals and infectious agents is an occupational risk; therefore, standard safety regulations must be maintained to avoid contamination.

IV. MINIMUM QUALIFICATIONS:

During the period of employment, shall be and remain a member of the QMC medical staff in good standing with appropriate privileges without restriction.
